IT Equipment/Media Services (ITEMS)
Request Form

This form is to be used to reserve any Information Technology Equipment or Media Services. This includes items such as any Notebook Personal Computers and all Audio/Visual equipment. Once you receive the equipment, it is your responsibility to care for the equipment. If you find that the equipment is not working properly, please make note of the deficiencies and tell the ITEMS Staff via e-mail or leave a message at extension 2306. Thank you for your cooperation.

After completing and submitting this form a copy will be sent to the ITEMS office for processing, you will also receive a copy of this request at the email address you indicate below. If you do not receive this copy, email items@ltsg.edu and report the problem.
