There are a number of form formats you will encounter while accessing the LTSG web site. Below are basic instructions for dealing with each type of form.

PDF Forms

The most common type of form on the LTSG web site is and Adobe PDF form. If you do not have the Adobe Reader installed on you computer this free program may be downloaded directly from Adobe. You may interact with these forms in one of two ways. Some forms you will download, print and fill in manually. Other forms offer the ability to actually enter data into the form before you print it out. You may also save these forms and work on them over an extended period of time if you are unable to complete the form all at once. Once you have opened a form and begun to complete it you can save your current work by clicking on the "Save a Copy" button. You can then open this copy at anytime, without being connected to the internet and edit the information you have entered. When you have completed the form and if a signature is required, print the form, sign it and return it to the proper office. If the form does not require a signature you may send it as an attachment to the appropriate person.

Web Forms

You will also find a number of web based forms on the LTSG Web site that allow you to fill in information directly into a web page and then send it to the Seminary. This forms are generally self explanatory and required fields will normally be labeled with a red asterisk. (*)
